What it is, How it works

Hair testing is a recognized tool for detecting drug and alcohol consumption. Hair strands capture a long-term history of substances due to biomarkers being embedded in growing hair fibers. A sample collected near the scalp can reveal up to an approximate 3-month look-back period for substances. Collecting hair is straightforward, difficult to alter, and easy to transport.

A 1.5-inch section containing about 200 hair strands (roughly the size of a #2 pencil) nearest the scalp provides 100mg, the ideal amount for screening and validation. For testing involving EtG, or more than 10 panels, a specimen of 150mg is advised. We suggest weighing the hair on a precise scale. If head hair isn't available, body hair can be used as an alternative. "Head hair" pertains only to scalp hair, while "body hair" includes all other varieties (facial, axillary, etc.).

Process Overview

The core phases in laboratory processing for a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is processed into a laboratory's system. This step ensures the sample's seal and shipment integrity, assigns it an LAN (Laboratory Accessioning Number), and completes any missing data entry apart from what the electronic chain of custody system provides.

Screening is a preliminary check for drugs of abuse. Although it's a cost-efficient way to dismiss drug usage in most cases, a positive screen must be confirmed for court acceptance. Any samples testing presumptively positive during Screening need a secondary confirmation.

If a sample shows presumptive positivity at Screening, extra hair from the initial sample is used for Extraction. Here, drugs are pulled from the hair at a much lower concentration than other methods (such as urine or oral fluid), which makes hair drug screening a challenging methodology to execute.

Confirmation of any positive screen occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are cleansed before confirmation if necessary. The end-to-end lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair program and accredited according to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
