A hair specimen of 1.5 inches, consisting of about 200 strands (approximately the size of a #2 pencil) closest to the scalp, will yield 100mg of hair, the perfect sample size for analysis and verification. For EtG, additional analysis, and/or tests over 10 panels, a 150mg sample is advised. We suggest measuring the specimen on a jeweler’s scale. If scalp hair is absent, an equivalent quantity of body hair may be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair refers to all other hair categories (facial, axillary, etc.).

Process Overview

The lab processing of a drug test includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample into a lab’s system. This involves checking that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary rapid test for drugs of abuse. While Screening is a budget-friendly method to exclude drug usage in most samples, a positive screen must be confirmed to be valid in court. Any samples that test positive in Screening require further confirmation.

If a sample tests positive in the Screening step, additional hair is taken from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

To verify any positive screening result, GC/MS, GC/MS/MS, or LC/MS/MS is used. All presumptive positive samples are cleaned before confirmation as needed. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
