What it is, How it works

Hair offers a long-term overview of alcohol and drug use by incorporating biomarkers within the fibers of the hair strand as it grows. When collected near the scalp, hair can offer a detection window of nearly 3 months for alcohol and drugs. Hair collection is easy, challenging to tamper with, and convenient to send.

A 1.5-inch segment consisting of approximately 200 hair strands (similar in size to a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, add-ons, and/or tests over 10 panels, a specimen of 150mg is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be used. When mentioning head hair, we mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample within a laboratory's system. This step includes confirming the sample's seal and proper shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not captured by an electronic chain of custody system.

Screening conducts an initial swift check for substances of abuse. While Screening is a cost-efficient method to exclude drug usage for most samples, a positive result must be confirmed for court admissibility. Any samples with a presumptive positive in Screening require secondary confirmation.

Should a sample exhibit a presumptive positive during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are extracted from hair at concentrations much lower than those in other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive Screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. The complete laboratory procedure, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
