A sample of 1.5 inches consisting of around 200 strands of hair (roughly the thickness of a #2 pencil) near the scalp will provide 100mg of hair, which is ideal for analysis and validation. For EtG, additional tests, and/or evaluations over 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for weighing the specimen is suggested. In the absence of scalp hair, a comparable quantity of body hair may be used. When mentioning head hair, we solely refer to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary phases in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a specimen into a laboratory’s intake. This involves ensuring the sample was correctly sealed and dispatched, generating a unique LAN (Laboratory Accessioning Number), and completing any auxiliary data entry absent from an electronic custody chain.

Screening provides an initial rapid assessment for drugs of abuse. While Screening is an economical way to eliminate drug use in most specimens, a positive screen necessitates confirmation for court admissibility. Presumptive positive samples in Screening require subsequent verification.

Should a sample show a presumptive positive in Screening, further hair from the original specimen is extracted and prepared for the Extraction phase. At this stage, substances are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

Verification of any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Presumptive positive samples are cleaned before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
