In the absence of scalp hair, an equivalent quantity of body hair can be used. When discussing head hair, we specifically mean scalp hair. The term body hair encompasses all other hair types (such as facial or axillary).

Process Overview

The core stages in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with processing a sample into the laboratory's database. This involves verifying proper sealing and shipment of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody.

Screening is an initial expedited assessment for drug presence. Although an effective way to negate drug usage on most samples cost-efficiently, a positive indication must be verified for court validity. Any sample showing a presumptive positive in Screening undergoes further confirmation.

If Screening suggests a positive result, additional hair is sourced from the initial sample and readied for Extraction. This step extracts drugs from the hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which renders hair drug screening particularly challenging.

Confirmation of any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples are washed before confirmation if necessary. The laboratory process from Accessioning through Confirmation is subject to evaluation under the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards compliance.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
