Collecting a 1.5-inch segment of approximately 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is an optimal sample for screening and confirmation. For testing that includes EtG, additional panels, or tests exceeding 10 panels, 150mg of hair is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not an option, an equivalent quantity of body hair may be used. When we mention head hair, we specifically mean scalp hair. The term body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary integration of a sample into a laboratory’s system. This step confirms that the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and addresses any additional data entry not captured by an electronic chain of custody system.

Screening provides a swift preliminary analysis to detect drugs of misuse. While Screening is cost-efficient for dismissing drug usage in most samples, a positive result from the screen must be verified for it to be recognized in legal settings. Samples that presumptively return a positive result in Screening must undergo a second round of confirmation.

In cases where a sample yields a presumptive positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methodologies (such as urine or oral fluids), which contributes to hair drug screening being one of the most challenging methodologies to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before undergoing confirmation when necessary. The entire lab workflow, from Accessioning through Confirmation, is rigorously evaluated according to the CAP (College of American Pathologists) Hair designation and is accredited under ISO / IEC 17025 standards.
