Hair can hold a prolonged history of these substances by capturing biomarkers within the strands' fibers. Obtaining hair samples close to the scalp offers up to a 3-month detection timeframe for drug and alcohol presence. Collecting hair is straightforward, somewhat resistant to tampering, and simple to mail.

A sample of 1.5 inches containing roughly 200 hair strands (comparable to a #2 pencil thickness) nearest to the scalp ensures 100mg of hair, an optimal amount for both screening and confirmation. For EtG add-ons and/or tests beyond 10 panels, 150mg of hair is advisable. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair is not an option, a comparable amount of body hair may be used. When mentioning head hair, this refers specifically to scalp hair. Body hair pertains to all other types (like facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample's initial integration into the lab system. This step includes ensuring the sample was adequately sealed and transported, assigning a LAN (Laboratory Accessioning Number), and managing any additional data input not covered by an electronic chain of custody system.

Screening is an initial rapid analysis for substances of abuse. Although cost-effective for eliminating drug usage in most samples, a positive result must be verified to be court-admissible. Samples with presumptive positives during Screening require a secondary confirmation.

For samples with presumptive positives in Screening, additional hair is sourced from the initial sample for Extraction. This process involves extracting drugs from hair in low concentrations unlike other techniques (such as urine or oral fluid), which makes hair drug screening particularly challenging to execute.

Confirmation of any positive screen result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are cleansed as necessary. The entire laboratory sequence from Accessioning to Confirmation is evaluated under both the College of American Pathologists (CAP) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
