For accurately screening drug use within the past 5 days, urine tests remain supreme.

What it is, How it works

Hair analysis is regarded as an effective method for identifying alcohol and drug consumption. Hair captures a long-term record of these substances by storing biomarkers within its growing strands. When taken from near the scalp, hair may offer a detection window of up to approximately three months for alcohol and other drugs. Its collection is straightforward, tampering is somewhat challenging, and shipment is simple.

A 1.5-inch section comprising approximately 200 hair strands (similar in size to a standard #2 pencil) nearest to the scalp yields around 100mg, which is optimal for both screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, 150mg of the sample is advisable. Using a jeweler’s scale for weighing the sample is recommended. If scalp hair is unavailable, the equivalent amount of body hair can be collected. 'Head hair' refers specifically to scalp hair, while 'body hair' covers all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample's initial processing in the lab's system, which includes verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening conducts an initial rapid assessment for drugs of abuse. Though cost-effective for excluding drug use in many samples, a positive screen must be confirmed to be permissible in court. Any samples showing a presumptive positive during Screening require further confirmation.

If Screening suggests a positive result, additional hair is extracted from the original sample for the Extraction phase, where drugs are retrieved from the hair at lower concentrations than other methods (e.g., urine, oral fluid). This complexity makes hair drug screening the most challenging method.

Confirmation of any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation, as necessary. From Accessioning to Confirmation, the entire process is reviewed under both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
