What it is, How it works

A 1.5-inch hair sample comprising around 200 strands (approximately the dimension of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred quantity for both screening and confirmation. For EtG add-ons and/or exams exceeding 10 panels, a specimen of 150mg is advisable. Utilizing a jeweler’s scale to weigh the sample is recommended. Should scalp hair be unavailable, a comparable quantity of body hair can be obtained. When referencing head hair, it specifically pertains to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary integration of a sample within a lab’s system. This encompasses confirming that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing any extra data entry not covered by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of abuse. Although Screening provides a cost-efficient method to rule out drug use for most samples, a positive screen must be verified to be legally acceptable in court. Samples yielding a presumptive positive in Screening necessitate a subsequent confirmation.

If the Screening stage indicates a presumptive positive, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at a much lower concentration than in alternative methods (ex. urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation for any positive screening outcomes is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo cleansing prior to confirmation as required. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
