Yet, for the most recent drug use within 5 days, urine tests remain accurate.

What it is, How it works

Hair analysis has emerged as an effective instrument for identifying drug and alcohol consumption. Hair offers a prolonged timeline of drug and alcohol use by entrapping biomarkers within the strands of hair as they grow. When gathered near the scalp, hair can deliver up to about a 3-month detection timeline for drugs and alcohol use. Hair collection is straightforward, somewhat resistant to tampering, and convenient for shipping.

A 1.5-inch segment of roughly 200 hair strands (comparable to a size #2 pencil) from near the scalp will yield 100mg of hair, which is the perfect sample size for initial and detailed analysis. For EtG, add-ons, and/or assessments beyond 10 panels, 150mg of hair is advisable. We suggest measuring the hair sample on a jeweler’s scale. If scalp hair is insufficient, an equal mass of body hair may be collected. When mentioning head hair, we specifically mean scalp hair. Body hair entails all other hair types (facial, axillary, etc.).

Process Overview

There are four key phases involved in the laboratory workflow of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial input of a sample into a laboratory’s framework. This includes confirming that the sample was securely sealed and properly dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra information entry that was not supplied by an electronic custody system.

Screening provides a preliminary rapid assessment for drug abuse. While Screening is an economical method for excluding drug use in most samples, a positive screen necessitates confirmation to be valid in legal proceedings. Any samples that test positive presumptively during Screening require secondary verification.

Should a sample display presumptive positivity in the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which is why hair drug testing is considered the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. Presumptively positive samples undergo washing before confirmation if necessary. The whole lab procedure from Accessioning to Confirmation is reviewed according to the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
