Collected near the scalp, hair can provide a detection window of up to roughly 90 days for alcohol and drugs. Hair is straightforward to collect, challenging to tamper with, and simple to transport.

A 1.5-inch segment consisting of around 200 hair strands (approximately the thickness of a #2 pencil) nearest the scalp provides 100mg of hair, which is ideal for both screening and confirmation. For EtG, supplementary tests, and/or panels exceeding 10, a sample amount of 150mg is advised. We suggest using a jeweler’s scale for weighing. In the event scalp hair cannot be obtained, a comparable quantity of body hair is an acceptable substitute. Our reference to head hair pertains solely to scalp hair, while body hair encompasses any other type (facial, axillary, etc.).

Process Overview

Four primary phases in the laboratory pathway for processing drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into the lab's system. This step verifies the sample was sealed and delivered correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any lacking data entry that wasn't included in the electronic chain of custody system.

Screening is a preliminary rapid test for detecting drugs of misuse. While it is a cost-effective method to eliminate drug use in the vast majority of samples, only confirmed positives are admissible as evidence in court. Any samples that are preliminarily positive in Screening require further confirmation.

Should a sample test presumptive positive in Screening, additional hair is extracted from the original sample to prepare for Extraction. This phase involves extracting drugs from hair at far lower concentrations compared to other methods (e.g., urine, oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation, as necessary. The entire laboratory process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
