For EtG, additional tests, and/or assessments beyond 10 panels, a 150mg specimen is advised. Measuring the specimen on a jeweler’s scale is recommended. If scalp hair isn’t accessible, a similar amount of body hair can be collected. When referencing head hair, this specifically means scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test outcome in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within the laboratory system. This step confirms the sample’s proper sealing and shipping, the assignment of a random LAN (Laboratory Accessioning Number), and the completion of any informative data entry absent from an electronic chain of custody system.

Screening entails an initial rapid assessment for drug abuse. While Screening is a cost-effective approach to disprove drug usage for the majority of samples, a positive screen must be confirmed to be valid in legal settings. Samples deemed presumptively positive in Screening necessitate further confirmation.

If Screening identifies a sample as presumptively positive, more hair is extracted from the initial specimen and prepared for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (such as urine or oral fluid), explaining why hair drug screening is highly challenging to carry out.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation as required. The complete laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
