What it is, How it works

Hair testing is increasingly recognized as an effective means for detecting drug and alcohol consumption. Hair provides a lengthy record of alcohol and drug use by capturing biomarkers within the strands. When hair is cut near the scalp, it can offer a detection timeframe of up to three months for alcohol and drugs. Hair is straightforward to collect, moderately challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches, consisting of roughly 200 strands of hair (around the diameter of a #2 pencil) and taken closest to the scalp, yields 100mg of hair, the optimal sample for analysis. For tests involving EtG, additional panels, or those exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair isn't accessible, an equivalent volume of body hair can be used. References to head hair pertain solely to scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The drug testing lab process comprises four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a lab's system. It involves checking the sample for proper sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While cost-effective for ruling out drug use in many samples, a positive screen requires confirmation for court acceptance. Samples with a presumptive positive in Screening need further verification.

Should a sample indicate a presumptive positive in Screening, further hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are retrieved from hair at far lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug tests the most intricate testing method.

Positive screening results go through Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. As needed, all samples marked presumptive positive are washed before Confirmation. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.
