What it is, How it works

When the sample is close to the scalp, hair can provide nearly a three-month period for detecting drugs and alcohol. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment containing roughly 200 hair strands (approximately the size of a #2 pencil) near the scalp will yield 100mg of hair, which is the preferred sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not accessible, an equivalent amount of body hair may be collected. When mentioning head hair, we are specifically referring to scalp hair. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within a lab’s framework. This consists of checking that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not facilitated by an electronic chain of custody system.

Screening entails an initial rapid evaluation for abusive drugs. While Screening is an economical approach to exclude drug use in the majority of samples, a positive screen must be confirmed for admissibility in legal settings. Any samples that test presumptively positive in Screening need a follow-up confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen for preparation in the Extraction phase. At this point, drugs are withdrawn from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive outcome from screening is validated by way of GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The complete lab sequence from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
