What it is, How it works

Hair testing has become recognized as an effective method for detecting drug and alcohol consumption. Hair offers a comprehensive history of alcohol and other substances by getting biomarkers locked within the strands of the growing hair. When collected from near the scalp, hair can offer a detection period of up to approximately three months for alcohol and other substances. Hair collection is straightforward, somewhat resistant to tampering, and convenient for shipping.

A sample measuring 1.5 inches from around 200 hair strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen of 150mg is advised. It's best to measure the specimen on a jewelry scale. If scalp hair isn't obtainable, a similar quantity of body hair can be gathered. When we mention head hair, it pertains solely to scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The four primary stages in processing a laboratory drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the inaugural handling of a sample within a laboratory's function. This process confirms the sample's seal and proper shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry absent in an electronic chain of custody system.

Screening constitutes an initial swift inspection for substances of abuse. Although Screening is an economical method to negate substance usage in most samples, a positive screen must be validated for legal acceptability. Samples presupposed to be positive in Screening indeed require further confirmation.

If a sample is deemed presumptively positive in the Screening phase, more evidence is taken from the initial sample and readied for Extraction. Confirmation of any positive screen result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presupposed positive samples are cleansed before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
