A 1.5-inch segment of around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not available, an equivalent quantity of body hair may be gathered. “Head hair” refers specifically to scalp hair, while “body hair” covers all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result encompasses four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial processing phase where the sample is integrated into the laboratory system. This step includes verifying secure and proper shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of abuse. While Screening offers a cost-effective means of excluding drug use for the majority of samples, a positive screen must be validated for legal admissibility. Samples flagged as presumptively positive in Screening necessitate a subsequent confirmation.

For samples that test presumptively positive in Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This involves extracting drugs from hair at substantially lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcomes is executed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.
