Upon a presumptively positive result during Screening, additional hair is extracted from the original specimen for Extraction. Herein, drugs are extracted from hair at much lesser concentrations compared to other methods (e.g., urine or oral fluid), making hair drug analysis particularly challenging to execute.
For any positive screening result, Confirmation is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as needed before confirmation. The laboratory process from Accessioning through Confirmation undergoes review both under the CAP (College of American Pathologists) Hair designation and against the ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
