What it is, How it works

Hair captures a long-term record of substance use by embedding biomarkers within the strands of the growing hair. When taken from just above the scalp, hair can offer roughly a 3-month window for detecting drug and alcohol use. It is straightforward to collect, challenging to tamper with, and simple to mail.

A sample measuring 1.5 inches and consisting of approximately 200 hair strands (similar to a #2 pencil in diameter) closest to the scalp provides 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional testing, and/or tests exceeding 10 panels, a specimen size of 150mg is advised. We suggest weighing the specimen on a jeweler's scale. If scalp hair is not available, a corresponding amount of body hair can be used. When mentioning head hair, we mean only scalp hair. Body hair includes all other types (e.g., facial, axillary).

Process Overview

The four primary stages in the laboratory processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample within a laboratory's system. This process encompasses verifying that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening acts as an initial rapid assessment for drugs of abuse. While Screening serves as a cost-efficient method to eliminate the majority of samples for drug usage, a positive screen must be confirmed for legal admissibility. Every sample showing presumptive positivity in Screening does need secondary confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the initial sample for the Extraction stage. During this phase, drugs are extracted from hair at a much lower concentration relative to other methods (e.g., urine or saliva), making hair drug testing the most complex method to execute.

Confirmatory testing of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are cleansed before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
