When taken near the scalp, hair can offer a detection period of up to three months for alcohol and drugs. It is easy to collect, not easily tampered with, and straightforward to mail.

A 1.5-inch section containing roughly 200 strands of hair (akin to the thickness of a #2 pencil) near the scalp will yield 100mg of hair, which is ideal for both evaluation and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. Using a jeweler’s scale for measurement is suggested. If scalp hair is lacking, an equivalent amount of body hair is acceptable. For head hair, we specify scalp hair only. Body hair encompasses all other types (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test comprises four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage of integrating a sample into the lab system. This step ensures the sample's proper sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any supplemental data entry not handled by an electronic chain of custody protocol.

Screening is a preliminary assessment for illegal drugs. Although it offers a cost-efficient method to exclude drug use in most samples, a confirmed positive screen is required for legal proceedings. Samples that screen positive preliminarily must undergo further confirmation.

Should a sample be suspect at the Screening phase, additional hair is extracted from the sample and processed for Extraction. This stage removes drugs from hair at lower concentrations compared to other techniques (e.g., urine or saliva), making hair drug testing notably challenging.

Positive screens are verified through GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged positive are cleansed before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is audited according to the CAP (College of American Pathologists) Hair standards and conforms to ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
