A sample of hair measuring 1.5 inches and consisting of about 200 strands (approximately the thickness of a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal quantity for both preliminary and confirmatory testing. For EtG, additional tests, or those exceeding 10 panels, 150mg of hair is advised. It's recommended to weigh the sample using a jeweler’s scale. If head hair isn't available, a comparable amount of body hair can be taken. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other types (like facial and axillary hair).

Process Overview

The four essential steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of introducing a sample into a lab’s system. This step covers ensuring the sample is sealed and delivered correctly, generating a unique LAN (Laboratory Accessioning Number), and fulfilling any supplementary data input not supplied by an electronic chain of custody.

Screening is a preliminary examination for substances of abuse. Although Screening is an economical approach to negate drug use for most samples, a positive finding must be verified for court validity. Any samples that show a presumptive positive in Screening necessitate further confirmation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the original sample and prepped for Extraction. In this phase, substances are extracted from the hair at notably lower concentrations than other methods (e.g., urine or saliva), underscoring the complexity of performing hair drug screens.

Confirmation of any positive from Screening is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are cleansed before confirmation if required. From Accessioning through to Confirmation, the whole lab procedure is evaluated under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
