What it is, How it works

When hair is clipped near the scalp, it can offer a detection period of up to about three months for alcohol and other substances. Collecting hair is straightforward, relatively hard to tamper with, and convenient for shipping.

A sample of 1.5 inches, consisting of roughly 200 hair strands (around the diameter of a #2 pencil) closest to the scalp, yields 100mg of hair—optimal for assessment and confirmation purposes. For EtG, add-ons, and/or evaluations exceeding 10 panels, a recommendation of 150mg of the specimen is given. Using a jeweler’s scale for weighing is advisable. In cases where scalp hair isn't accessible, an equivalent measure of body hair can be gathered. When specifying head hair, we indicate only scalp hair. Body hair pertains to all other hair categories (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial placement of a sample into a lab’s framework. This phase checks the proper sealing and shipping of the sample, assigns an arbitrary LAN (Laboratory Accessioning Number), and concludes any additional data input not covered by an electronic chain of custody system.

Screening provides a preliminary rapid check for substance abuse. Although Screening effectively eliminates drug use suspicions from most samples at a lower cost, a positive screen requires confirmation for validity in legal contexts. Samples presumed positive in Screening necessitate a follow-up confirmation.

If Screening suggests a positive outcome, additional hair is sourced from the primary sample and set up for Extraction. This phase extracts drugs from hair at considerably lower concentrations than other techniques (e.g., urine or oral fluid), a factor that contributes to the complexity of hair drug screenings.

Positive results from screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumed positive samples are washed before confirmation when necessary. The full lab cycle from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair benchmarks and ISO / IEC 17025 accreditation criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
