For EtG, additional tests, and/or panels exceeding ten, a specimen amount of 150mg is advisable. It is suggested to measure the specimen using a jeweler’s scale. Should scalp hair be unavailable, an equivalent mass of body hair may be used. The term head hair specifically denotes scalp hair, whereas body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is composed of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the integration of a sample into a laboratory’s system. This process includes confirming the proper sealing and shipping of the sample, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any additional data entry absent in an electronic chain of custody protocol.

Screening serves as a preliminary rapid assessment for drug presence. While it is an economical method to eliminate drug usage in the majority of cases, any positive result from Screening must undergo confirmation to be deemed valid for legal proceedings. Samples yielding presumptively positive results in Screening require a subsequent confirmation.

Should a Screening stage result suggest drug presence, further hair is extracted from the initial sample and readied for Extraction. During this stage, drugs are withdrawn from hair at a considerably diminished concentration compared to other methods (e.g., urine or oral fluid), rendering hair drug screening a challenging technique to execute.

Confirmation for any screened positive result is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing presumptive positives are cleansed prior to confirmation as necessary. The entire lab procedure from Accessioning to Confirmation undergoes scrutiny under both CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
