Obtaining a 1.5-inch hair sample comprising roughly 200 strands, equivalent to the width of a #2 pencil, near the scalp yields 100mg, which is optimal for initial tests and validation. For more comprehensive analysis like EtG screenings or tests exceeding 10 panels, it's advisable to gather 150mg. Utilize a jeweler’s scale for precise measurement. If scalp hair is not accessible, a similar volume of body hair may be obtained. When mentioning head hair, we mean scalp hair specifically. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory drug testing procedure comprises four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with incorporating a sample into the lab system. This covers ensuring the sample's proper sealing and delivery, generating a unique LAN (Laboratory Accessioning Number), and executing any additional information entry not derived from an electronic chain of custody.

Screening encompasses a swift preliminary check for drug use. Although Screening is an affordable method to initially rule out drugs in most samples, confirmed evidence is needed for legal purposes. Any initially positive samples will require further Confirmation.

If a sample tests potentially positive during Screening, additional hair is isolated from the initial batch for Extraction. During this phase, drugs are retrieved from hair at much lower levels compared to other methods (e.g., urine or oral fluid), making hair testing the most challenging to perform.

Confirmation for any positive result from Screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positives are washed if necessary. From Accessioning to Confirmation, the whole process is evaluated under CAP (College of American Pathologists) Hair standards, along with ISO / IEC 17025 accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
