What it is, How it works

Hair analysis has become regarded as an effective method for identifying drug and alcohol consumption. Hair offers a long-term record of substance use by capturing biomarkers in the strands of growing hair. When sampled near the scalp, hair can reveal up to a three-month window of drug and alcohol usage. Hair collection is straightforward, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (similar in thickness to a #2 pencil) nearest the scalp provides 100mg of hair, the optimal sample for both initial screening and further confirmation. For EtG tests, additional panels, or tests exceeding 10 panels, 150mg of the specimen is advisable. Using a jeweler’s scale for weighing the specimen is suggested. If head hair is not available, an equivalent amount of body hair can be utilized. The term "head hair" specifically denotes scalp hair, while "body hair" includes all other hair varieties (facial, underarm, etc.).

Process Overview

The primary stages in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into the lab’s system, ensuring that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not covered by an electronic chain of custody system.

Screening performs an initial quick assessment for drugs of misconduct. Though Screening is an economical approach to dismiss drug usage in most samples, a positive screen requires confirmation to be legally admissible. Samples that screen as presumptive positives need subsequent confirmation.

Upon a presumptive positive result in the Screening stage, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than alternative methods (e.g., urine or saliva), which makes hair drug examination the most challenging method to execute.

All positive screening outcomes require Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a required washing before confirmation. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
