A sample taken near the scalp can offer up to about a 3-month window for detecting alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and convenient to post.

A specimen consisting of approximately 200 hair strands, cut to 1.5 inches (comparable to the size of a #2 pencil), will yield 100mg of hair, which is the optimal amount for initial testing and verification. For tests involving EtG, additional panels, or over 10 drug panels, a sample of 150mg is advisable. Utilizing a jeweler’s scale for specimen weighing is suggested. Should scalp hair be unavailable, a similar quantity of body hair can be collected. "Head hair" specifically refers to hair from the scalp. "Body hair" pertains to all other hair types (facial, axillary, etc.).

Process Overview

In drug test laboratory processing, the four primary stages include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step involving inputting a sample into the laboratory’s system. This step verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional necessary data input not captured by an electronic chain of custody system.

Screening is a preliminary review for substance abuse. Although this stage is a cost-efficient way to exclude drug use in most samples, a positive screen must be confirmed to be valid in legal settings. Samples flagged as presumptively positive during Screening necessitate secondary Confirmation.

For samples deemed presumptively positive at the Screening stage, additional hair is extracted from the initial sample and readied for Extraction. Confirmation of positive findings from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples deemed presumptively positive undergo washing before Confirmation, as required. The process, from Accessioning through Confirmation, is subjected to review under the CAP (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
