What it is, How it works

When acquired near the scalp, it can offer a detection time span of up to roughly three months for alcohol and substances. Hair is straightforward to harvest, relatively tough to tamper with, and straightforward to transport.

A segment measuring 1.5 inches with about 200 strands (approximately the width of a #2 pencil) close to the scalp provides 100mg of hair, which is the perfect amount for analysis and validation. For EtG, enhancements, or tests beyond 10 panels, it is advised to use 150mg of the sample. Using a jeweler’s scale to measure the sample is suggested. If scalp hair is not present, a comparable amount of body hair can be taken. The term head hair applies solely to scalp hair. All other hair types (facial, axillary, etc.) fall under body hair.

Process Overview

The laboratory analysis of a substance test result is broken down into four main procedures: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a specimen into a lab’s database. This ensures the specimen was properly sealed and dispatched, a unique LAN (Laboratory Accessioning Number) is allocated, and any extra data entry not supplied by an electronic chain of custody system is finalized.

Screening executes a quick preliminary assessment for drug misuse. Despite being a cost-effective approach to eliminate substances in most samples, a positive indication in Screening necessitates verification for legal acceptance. Any samples that test presumptively positive in Screening require a secondary validation.

If a sample is indicated as presumptively positive during Screening, additional hair is extracted from the initial sample for Preparation in Extraction. Drugs are retrieved from hair at a lower concentration compared to other techniques (e.g., urine or oral fluids), making hair drug screening the most challenging technique to execute.

Verification of any positive result from Screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to verification as necessary. The whole laboratory procedure from Accessioning to Confirmation is evaluated in accordance with both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
