A sample of 1.5 inches, consisting of roughly 200 hair strands (comparable in size to a #2 pencil) closest to the scalp, will yield 100mg of hair, which is the preferred amount for testing and validation. For EtG, additional assessments, and/or testing beyond 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale for weighing the sample is recommended. Should scalp hair not be available, a similar quantity of body hair might be used. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other types like facial or axillary hair.

Process Overview

The process of laboratory drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is introduced into the lab’s system. This step involves confirming the sample's proper sealing and shipping, assigning a Laboratory Accessioning Number (LAN) randomly, and executing any further data input not covered by an electronic chain of custody system.

Screening is a preliminary, rapid examination for substances of abuse. While it is cost-effective for excluding drug use in most samples, a positive result needs confirmation for legal admissibility. Samples that show a presumptive positive result in Screening require a secondary affirmation.

If a sample shows presumptive positivity during Screening, additional hair from the initial sample is prepared for Extraction. This phase involves retrieving drugs from hair at a much lower concentration than other methods, such as urine or oral fluids, which makes hair drug testing the most challenging method.

For confirming any positive screening results, techniques such as GC/MS, GC/MS/MS, or LC/MS/MS are employed. All samples with presumptive positive results undergo washing as necessary before confirmation. The comprehensive lab process from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
