What it is, How it works

Hair testing is now regarded as an effective method for identifying drug and alcohol consumption. Hair offers a prolonged history of alcohol and other drugs by embedding biomarkers within the strands of the growing hair. When taken from near the scalp, hair can offer an approximately 3-month detection period for alcohol and other drugs. Hair collection is straightforward, relatively resistant to adulteration, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (similar to the size of a #2 pencil) nearest the scalp yields 100mg of hair, which is the ideal sample size for assessment and verification. For EtG, add-ons, and/or tests that exceed 10 panels, 150mg of the sample is recommended. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't available, an equivalent amount of body hair can be gathered. When mentioning head hair, we refer only to scalp hair. Body hair denotes any other hair types (facial, axillary, and others).

Process Overview

The laboratory processing of a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab's system. It entails ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing any extra data entry absent from an electronic chain of custody system.

Screening involves an initial rapid test for drug abuse. Though Screening offers an economical way to exclude drug use in most samples, a positive screen must be confirmed for legal admissibility. Any samples that show presumptive positivity during Screening must undergo a secondary confirmation.

Should a sample exhibit presumptive positivity during the Screening step, additional hair is pulled from the initial specimen and readied for Extraction. During this phase, drugs are extracted from hair at a much lower concentration level than in other methods (e.g., urine or oral fluid), which is why hair drug screening is among the most challenging methods to execute.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
