However, urine tests remain the best choice for identifying drug use within the last 5 days.

What it is, How it works

Hair analysis has become recognized as an effective method for identifying drug and alcohol use. Hair captures a long-term profile of alcohol and drugs by embedding biomarkers within the fibers of the developing hair shaft. When sampled near the scalp, hair can offer up to an estimated 3-month period of detection for alcohol and various drugs. Hair collection is straightforward, challenging to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hairs (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for analysis and verification. For EtG, additional tests, and/or panels exceeding 10, it is advisable to have 150mg of the specimen. We suggest measuring the specimen using a jeweler’s scale. If head hair is not available, a similar volume of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair covers all other hair forms (facial, armpit, etc.).

Process Overview

The four primary stages in the laboratory procedure for a drug test outcome include Accessioning, Screening, Extraction, and Verification.

Accessioning consists of the initial administration of a specimen into a laboratory’s system. This step verifies if the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data input not included by an electronic chain of custody system.

Screening entails an initial swift assessment for substance abuse. While Screening is a budget-friendly means to exclude drug usage on most samples, a positive result must be confirmed to be acceptable in legal contexts. Any samples that test presumptively positive in Screening must undergo a secondary confirmation.

If a sample tests presumptively positive in the Screening phase, additional hair is obtained from the original specimen and prepared for Extraction. During this phase, drugs are withdrawn from hair at a significantly lesser concentration compared to other methods (e.g., urine or oral fluid), underscoring why hair drug analysis is the most complex methodology to execute.

Verification of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive specimens are cleansed before verification as necessary. The entire laboratory procedure from Accessioning to Verification is evaluated under both the CAP (College of American Pathologists) Hair specification and accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
