For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is advised. It's best to use a jeweler’s scale to weigh the specimen. If scalp hair is absent, a similar quantity of body hair may be collected. When mentioning head hair, this specifically refers to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory process for handling a drug test encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage where a sample is entered into the laboratory’s system. This step ensures the sample is properly sealed and shipped, assigns it a random LAN (Laboratory Accessioning Number), and completes any missing data not covered by an electronic chain of custody system.

Screening consists of an initial rapid assessment for drug presence. While Screening is a cost-effective method to eliminate drug use in most samples, a positive result needs further confirmation for legal admissibility. Samples that are preliminarily positive in Screening necessitate a secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is taken from the initial sample and prepared for Extraction. This stage involves extracting drugs from hair at concentrations lower than other methods (such as urine or oral fluid), which is why hair drug screening is among the most challenging techniques to conduct.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed beforehand if necessary. Every aspect of the laboratory process, from Accessioning to Confirmation, is thoroughly examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
