What it is, How it works

Hair captures a prolonged history of substances by embedding biomarkers within the fibers of the maturing hair strand. When cut close to the scalp, hair offers a detection period of up to roughly three months for substances. Hair collection is straightforward, less easily tampered with, and convenient to transport.

A 1.5-inch segment comprising approximately 200 hair strands (similar in size to a #2 pencil) near the scalp provides 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, supplementary tests, and/or panels exceeding 10, it is advised to use 150mg of the hair sample. Utilizing a jeweler’s scale for weighing the sample is recommended. Should scalp hair be unobtainable, an equivalent quantity of body hair can be gathered. In reference to head hair, we specify only scalp hair. Body hair includes all other hair categories (such as facial and axillary).

Process Overview

In the laboratory processing of drug test results, four principal stages are involved: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a specimen into a laboratory’s framework. This process involves verifying proper sealing and shipping of the specimen, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not facilitated by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. Although Screening is a cost-efficient method to dismiss drug usage in most samples, a screening positive result requires confirmation for court admissibility. Any samples that show a presumptive positive outcome during Screening necessitate further confirmation.

Should a sample be presumptively positive in the Screening phase, additional hair is removed from the initial sample for preparation in Extraction. During this phase, drugs are extracted from hair at reduced concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
