Hair sample collection is uncomplicated, relatively resistant to tampering, and straightforward to transport.

A 1.5-inch portion comprising roughly 200 hair strands (about the diameter of a #2 pencil) near the scalp yields 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional testing, and/or over 10-panel tests, 150mg is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair isn’t available, a comparable quantity of body hair can be taken. When we mention head hair, we specifically refer to scalp hair. Body hair denotes all other types (facial, axillary, etc.).

Process Overview

The core steps in laboratory testing of drug samples are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to initially processing a sample into the lab’s infrastructure. This step includes confirming the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and concluding any extra data entry not handled by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug misuse. While Screening is cost-efficient for eliminating drug use in the bulk of samples, a positive outcome must be confirmed for courtroom acceptance. Samples that test presumptively positive during Screening require further confirmation.

If a specimen is tentatively positive in the Screening phase, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are isolated from hair at much lesser concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening presents the highest level of complexity.

Verification of any positive screen result is done through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing prior to confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
