What it is, How it works

Hair analysis has become recognized as an effective method for detecting drug and alcohol consumption. The hair strand traps biomarkers within its fibers, offering an extensive historical record of alcohol and drug use. When samples are taken close to the scalp, they can provide a detection window of up to approximately three months for alcohol and other drugs. The collection process for hair is straightforward, making it hard to tamper with and convenient to transport.

A sample of 1.5 inches consisting of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for initial screening and later confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of hair is advised. It is suggested to measure the sample using a jeweler’s scale. If scalp hair can't be collected, an equivalent quantity of body hair may be utilized. "Head hair" specifically refers to scalp hair. "Body hair" encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into a lab’s system. This step ensures that the sample is correctly sealed and shipped, assigns a random Laboratory Accessioning Number (LAN), and completes any remaining data entry not covered by an electronic chain of custody system.

Screening performs an initial quick assessment for drugs of abuse. Though Screening is cost-efficient for ruling out drug use in most samples, a positive screening must be confirmed to be legally admissible. Any samples with presumptive positive results from Screening require further verification.

If Screening results are presumptively positive, additional hair is extracted from the initial sample for the Extraction stage. In this step, drugs are retrieved from hair at lower concentrations than other methods (such as urine or oral fluid), which is why hair drug screening is considered the most challenging methodology.

Confirmation for any positive screen is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples undergo washing before confirmation as necessary. The full lab process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation and aligns with the accreditation of ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
