What it is, How it works

A 1.5-inch sample, consisting of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is considered optimal for screening and confirmation purposes. For EtG, additional testing, and/or panels exceeding 10, a specimen weight of 150mg is advised. It's recommended to weigh the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair can be obtained. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps in laboratory analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample within the laboratory system, including verifying the sample’s secure sealing and proper shipment, assigning a unique LAN (Laboratory Accessioning Number), and carrying out any supplementary data entry absent in an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug presence. While Screening is an economical approach to eliminate drug use for most samples, a positive result must be confirmed for legal admissibility. Presumptive positives in Screening necessitate an additional confirmation.

If a sample shows a presumptive positive in the Screening phase, more hair is extracted from the initial specimen for Extraction. This phase involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug testing.

Confirming any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo washing before confirmation if required. The full lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
