A sample of 1.5 inches consisting of approximately 200 hair strands (equivalent to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect quantity for both screening and confirmation purposes. For tests that include EtG, additional panels, or exceed 10 categories, a specimen amount of 150mg is suggested. We advise weighing the sample using a jeweler’s scale. If scalp hair is not an option, the same amount of body hair can be obtained. "Head hair" refers exclusively to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the commencement of integrating a sample into a lab’s system. This stage ensures the sample is sealed and dispatched correctly, allocates a random LAN (Laboratory Accessioning Number), and concludes any supplementary data entry absent in an electronic chain of custody system.

The Screening phase includes an initial rapid assessment for drug misuse. Although Screening is a cost-efficient method to eliminate drug use in the majority of samples, a positive result from screening requires confirmation for legal acceptability. Samples that show presumptive positives in Screening necessitate further confirmation.

If a sample is deemed presumptively positive during Screening, additional hair from the original specimen is extracted and readied for the Extraction phase. Here, drugs are extracted from hair at substantially lower concentrations than in alternative methods (such as urine or oral fluid), making hair drug screening the most intricate methodology.

Any positive results from the Screening phase undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation along with ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
