A sample measuring 1.5 inches from roughly 200 hair strands (approximately the diameter of a #2 pencil) cut nearest to the scalp will yield 100mg of hair, which is the preferred quantity for analysis and validation. For EtG, supplementary, and/or tests beyond 10 panels, a 150mg sample is suggested. It's advisable to weigh the sample with a jeweler’s scale. Should scalp hair be unavailable, an equivalent amount of body hair can be used. When referencing head hair, we specify scalp hair exclusively, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the lab comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary handling of a sample within a lab's framework. This step includes confirming the sample's secure sealing and proper dispatch, allocating a random LAN (Laboratory Accessioning Number), and finalizing any outstanding data input not facilitated by an electronic chain of custody system.

Screening conducts an initial assessment for substance misuse. While Screening is an economical method for excluding drug use in most samples, a positive result necessitates further verification to be legally admissible. Samples flagged as presumptively positive during Screening require subsequent confirmation.

When a sample is flagged as presumptively positive at the Screening phase, additional hair is extracted from the initial sample and set aside for Extraction. At this juncture, drugs are extracted from hair at remarkably lower concentrations than in alternate methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes washing before confirmation as warranted. The entire laboratory process, spanning from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
