A 1.5-inch strand collection comprising roughly 200 hairs (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect amount for evaluating and confirming. For EtG, additional analyses, and/or assessments exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample on a jeweler's scale. If head hair isn't available, an equivalent quantity of body hair may be used. When mentioning head hair, we mean scalp hair specifically. Body hair includes all other types (facial, axillary, etc.).

Overview of the Process

The laboratory processing of a drug test follows four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a laboratory's framework. This entails verifying the sample's seal and proper shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data recording not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is an economical way to exclude drug usage in most cases, a positive screen must be verified to be considered valid in court. Samples that show a positive indication in Screening require further confirmation.

If a sample gives a positive indication during the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. During Extraction, drugs are isolated from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most complex method to execute.

Any positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The full laboratory process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
