Zero-tolerance employers, judicial systems, and those on probation utilize these tests for their extensive drug detection window, typically up to 90 days, and their resistance to tampering, although urine tests remain more effective for recent drug use.

What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol use. Hair captures a long-term history of alcohol and drugs by embedding biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer up to about a 3-month detection period for alcohol and other substances. Hair is easy to collect, relatively hard to tamper with, and simple to send.

A 1.5-inch sample consisting of approximately 200 hair strands (equivalent to the size of a #2 pencil) nearest the scalp will provide 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If there is no scalp hair available, an equivalent amount of body hair can be used. When referring to head hair, we mean scalp hair exclusively. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps involved in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample into a laboratory’s system. This includes ensuring the sample was sealed and sent properly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not provided by an electronic chain of custody system.

Screening involves an initial rapid test for drugs of abuse. Although Screening is an economical way to exclude drug use in most samples, a positive screen requires confirmation to be valid in court. Any samples that are presumptively positive in Screening do require a secondary confirmation.

If a sample is presumptively positive in the Screening stage, additional hair is extracted from the initial sample and prepped for Extraction. In this process, drugs are drawn from hair at much lower levels than by other methods (such as urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Note: Though commonly named "hair follicle tests", the examination itself assesses the hair strand, not the follicle beneath the scalp.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
