Hair collection is uncomplicated, somewhat resistant to tampering, and straightforward to transport.

A segment measuring 1.5 inches containing roughly 200 hair strands (about the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for testing and verification. For EtG, supplemental analysis, and/or tests exceeding 10 panels, a sample of 150mg is advised. We suggest measuring the sample weight on a jeweler’s scale. If scalp hair is inaccessible, an equivalent quantity of body hair can be used. "Head hair" refers solely to scalp hair, while "body hair" encompasses all other types (beard, armpit, etc.).

Process Overview

The laboratory analysis process of a drug test comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial processing of a specimen within a lab’s infrastructure. This step ensures the sample was appropriately sealed and transported, assigns it a unique LAN (Laboratory Accessioning Number), and finalizes any outstanding data entry not catered for by an electronic chain of custody system.

Screening is a preliminary check for substance abuse. As an economic approach to exclude drug presence in the majority of samples, a positive result from Screening must be verified to be court-admissible. Presumptive positive samples in Screening demand subsequent confirmation.

When Screening indicates a presumptive positive result, further hair is extracted from the original sample and readied for Extraction. At this phase, drugs are isolated from hair at much lower concentrations than in alternatives such as urine or oral fluid, explaining why hair drug analysis is the most intricate methodology to execute.

Validation of any positive results from screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS methods. Each presumptive positive sample undergoes washing before confirmation as needed. The entire review process from Accessioning to Confirmation falls under the regulations of the CAP (College of American Pathologists) Hair designation as well as accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
