Despite this advantage, urine tests remain the gold standard for detecting recent drug use within the last five days.

What it is, How it works

Hair testing is now widely recognized as an effective method for detecting drug and alcohol consumption. The hair captures a long-term record of alcohol and drug use by imprisoning biomarkers within the strands as they grow. When hair is sampled near the scalp, it can reveal a detection period of up to around three months for drugs and alcohol. Hair sampling is straightforward, relatively hard to tamper with, and convenient for transportation.

A 1.5-inch segment containing roughly 200 hair strands (equivalent to the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal amount needed for both screening and confirmation tests. To perform EtG, add-ons, or tests exceeding 10 panels, 150mg of hair is advised. It's suggested to use a jeweler’s scale for measuring. If hair from the scalp is unavailable, an equal amount of body hair is an alternative. Head hair refers specifically to scalp hair, while body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory process for drug testing can be divided into four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample as it enters a laboratory's workflow. This step involves ensuring the sample was correctly sealed and sent, assigning it a unique Laboratory Accessioning Number (LAN), and completing any supplementary data input not received via an electronic chain of custody.

Screening is a preliminary evaluation to check for substances of abuse. Although Screening is a cost-efficient approach for eliminating drug use in most samples, a positive result needs to be verified before it’s taken as credible in legal settings. Any sample with a presumed positive outcome in Screening needs a follow-up confirmation.

For samples testing presumptively positive in the Screening phase, additional hair is drawn from the original sample for Extraction. In this stage, substances are extracted from the hair at notably lower concentrations than seen in other testing methods (such as urine or oral fluid), which is why hair drug testing is among the most challenging procedures.

Confirmation for any positive Screening outcomes is done through GC/MS, GC/MS/MS, or LC/MS/MS. Every presumed positive sample is cleansed before confirmation as needed. The full laboratory process, from Accessioning to Confirmation, is examined under the CAP (College of American Pathologists) Hair criteria alongside the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
