What it is, How it works

Hair analysis has gained recognition as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the hair as it grows. A sample taken near the scalp can offer a detection window spanning up to around three months for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and convenient to transport.

A sample of 1.5 inches comprising approximately 200 hair strands (similar to the size of a #2 pencil) closest to the scalp will provide 100mg of hair, the optimal quantity for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's suggested to use a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent quantity of body hair can be utilized. By "head hair," we mean scalp hair only. "Body hair" encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial integration of a sample into a lab’s system. This process involves checking that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid test for drugs of abuse. Though Screening is an economical method to rule out drug use in most cases, a positive screen must be confirmed for legal admissibility. Samples that are presumptively positive in Screening must undergo a secondary confirmation.

When a sample tests presumptively positive in the Screening phase, further hair is retrieved from the initial sample and readied for Extraction. Here, drugs are removed from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most complex methodologies to execute.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The whole laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
