A 1.5-inch segment comprising around 200 hair strands (approximately the diameter of a #2 pencil) taken from the scalp provides 100mg of hair, which is the optimal amount for analysis and verification. For EtG, additional examinations, and/or tests exceeding 10 panels, 150mg of hair is suggested. It is advisable to measure the hair sample with a jeweler's scale. If scalp hair is absent, a comparable quantity of body hair can be utilized. In terms of head hair, we specifically mean scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The main laboratory steps in processing a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step for incorporating a sample into a lab's system. It involves confirming that the sample is properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and filling in any additional information not included by an electronic chain of custody system.

Screening is a preliminary assessment for the presence of drugs. While Screening is an economical approach for eliminating drug usage on most samples, a positive result requires further confirmation to hold up in a legal setting. Samples testing positive in Screening will need a secondary verification.

Should a sample test positive during the Screening stage, more hair is extracted from the original sample and prepared for Extraction. During Extraction, drugs are removed from the hair at a considerably lower concentration compared to other techniques (such as urine or oral fluid), making hair drug screening a challenging method to execute.

Confirmation for any positive result in screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before any sample is confirmed as positive, it is washed as necessary. The full laboratory procedure, from Accessioning to Confirmation, is scrutinized to meet both the CAP (College of American Pathologists) Hair specification and ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
