Should scalp hair be unavailable, a comparable quantity of body hair may be acquired. Referring to head hair denotes scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Procedure Outline

The four primary stages in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample within a laboratory’s framework. This step involves confirming the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry absent from an electronic chain of custody system.

Screening conducts an initial rapid evaluation for drug abuse. Screening serves as a cost-efficient method to rule out drug usage in most samples; however, a positive screening requires confirmation to retain court validity. Samples with presumptive positive results in Screening necessitate a secondary verification.

For samples deemed presumptively positive in the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. This phase involves extracting drugs from hair at considerably lower concentrations than other methodologies (e.g., urine or oral fluid), illustrating why hair drug screening stands as the most challenging methodology to execute.

Confirmation of any positive screening result utilizes GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing prior to confirmation as needed. The comprehensive laboratory process from Accessioning to Confirmation is appraised under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
