What it is, How it works

When taken from near the scalp, hair can reveal up to roughly a 3-month duration of alcohol and drug detection. Hair collection is straightforward, moderately resistant to tampering, and easy to transport.

A sample measuring 1.5 inches from about 200 hairs (around the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the preferred amount for both screening and confirmation. For tests involving EtG, additional components, and/or exceeding 10 panels, 150mg of the hair specimen is advised. It's suggested to use a jeweler’s scale for weighing the specimen. If scalp hair isn't available, an equivalent amount of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair includes all other types of hair (facial, underarm, etc.).

Process Overview

The process consists of four primary steps in laboratory handling of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing step in integrating a sample into a laboratory’s system. It includes ensuring the sample's seal and shipment were secure, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry from an electronic chain of custody system.

Screening serves as a rapid preliminary test for drugs of misuse. Although Screening offers a cost-effective way to exclude drug usage in most samples, a positive screen must be verified for legal acceptability. Samples that are presumptively positive in Screening need secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for further Extraction. At this phase, drugs are removed from the hair at a lower concentration than with other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

A positive screen result is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are cleansed before confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
