What it is, How it works

Hair acts as a repository for long-term consumption patterns by embedding biomarkers within the strands as they grow. When collected from near the scalp, hair can offer a detection window of up to roughly three months for alcohol and other substances. Hair sample collection is straightforward, hard to tamper with, and convenient to transport.

A sample of 1.5 inches consisting of around 200 hair strands (comparable to a #2 pencil width) near the scalp yields about 100mg, which is the optimal sample size for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, a sample of 150mg is advisable. It is recommended to use a jeweler’s scale to measure the sample weight. If scalp hair is unavailable, an equivalent amount of body hair can be gathered. By head hair, we exclusively mean scalp hair. Other types of hair (such as facial, axillary, etc.) are classified as body hair.

Process Overview

The drug test laboratory process involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first processed into the lab’s system. This step includes checking that the sample was adequately sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody system.

Screening includes an initial rapid assessment for illicit substances. Although Screening is an economical way to dismiss drug usage for most samples, a positive result needs further verification for legal admissibility. Any samples posing as positive during Screening require additional confirmation.

Should a sample test as possibly positive in the Screening phase, more hair is extracted from the sample for the Extraction phase. Here, substances are isolated from hair at a much finer concentration compared to other methods (such as urine or oral fluid), which makes hair drug testing the most complex methodology.

Confirmation of any positive result from Screening is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is cleaned prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation undergoes review in compliance with both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
