What it is, How it works

Hair testing is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the fiber of the growing hair strand. When taken close to the scalp, hair offers a detection window of up to about 3 months for alcohol and other drugs. Collection of hair is straightforward, challenging to adulterate, and easy to transport.

A sample size of 1.5 inches, consisting of roughly 200 hair strands (similar to a #2 pencil's thickness) nearest to the scalp, provides 100mg, which is the optimal sample size for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample amount of 150mg is advised. It is recommended to weigh the sample using a jeweler's scale. If scalp hair isn’t an option, an equivalent volume of body hair can be used. Reference to head hair specifically means scalp hair. Body hair refers to any other type (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the phase where a sample is initially processed into a lab's system. This involves confirming that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While cost-efficient in ruling out drug use for most samples, a positive screening result needs confirmation for court admissibility. Any samples yielding a presumptive positive during Screening must undergo secondary confirmation.

If Screening results in a presumptive positive, more hair is extracted from the original sample and readied for Extraction. During this phase, drugs are extracted at a significantly lower concentration from hair compared to other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging to perform.

Confirmation of a positive screen is done via GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptively positive are washed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
