A 1.5-inch sample consisting of roughly 200 hair strands (about the circumference of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for examination and verification. For EtG, supplementary tests, and tests exceeding 10 panels, 150mg of hair is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be obtained. When discussing head hair, we mean solely scalp hair. Body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test sample encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory’s framework. This process confirms the sample’s seal and correct shipping, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry absent from an electronic chain of custody setup.

Screening entails a preliminary rapid examination for drugs of abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a screen indicating positivity must undergo confirmation to be accepted in legal settings. Any samples showing potential positivity in Screening necessitate further confirmation.

If a sample shows potential positivity during Screening, additional hair is retrieved from the primary sample and made ready for Extraction. In this phase, drugs are extracted from hair at a lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening particularly challenging.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing presumptive positivity are cleansed before confirmation as necessary. The comprehensive laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
