A specimen measuring 1.5 inches, encompassing around 200 hair strands (similar in girth to a #2 pencil), nearest the scalp will yield 100mg of hair—ideal for both screening and confirmation. For EtG, additions, and/or tests beyond 10 panels, a 150mg sample is recommended. It is advised to use a jeweler's scale to weigh the sample. If scalp hair is not available, an equivalent amount of body hair is acceptable. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The main steps in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into the lab's system. This includes checking proper seal and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional information not covered by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of misuse. Screening is cost-efficient for ruling out drug use in most samples, but a positive screen requires confirmation to stand in court. Presumptively positive samples in Screening need a secondary confirmation.

If Screening suggests a positive result, more strands are extracted from the original sample for Extraction. This phase isolates drugs from hair at much lower concentrations than alternate methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of any positive screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleansed before confirmation when necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair designation and meets ISO / IEC 17025 accreditation standards.
