A sample measuring 1.5 inches and containing around 200 hair strands (comparable in size to a #2 pencil) taken close to the scalp will yield 100mg of hair, which is optimal for both screening and confirmation. For additional tests such as EtG and tests beyond 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, collecting an equivalent amount of body hair is an option. When mentioning head hair, we specifically mean scalp hair. In contrast, body hair pertains to all other hair types, including facial and axillary hair.

Process Overview

The processing of a drug test result in the laboratory involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial stage of integrating a sample into a laboratory system. The process includes verifying the correct sealing and shipping of the sample, assigning it a unique LAN (Laboratory Accessioning Number), and filling in any missing data not captured by an electronic chain of custody.

Screening entails a preliminary assessment for substances of abuse. Though it serves as a cost-effective measure to identify drug usage in most samples, a positive screen needs validation to be legally valid in court. Samples that yield presumptively positive results during Screening require further confirmation.

Should a sample show a presumptive positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. Confirmation of a positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with presumptive positives are cleaned as necessary. Each segment of the laboratory procedure from Accessioning to Confirmation is subject to evaluation under the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
