For EtG, add-ons, and/or tests beyond 10 panels, a 150mg sample is suggested. It's advisable to weigh the hair sample using a jeweler’s scale. If hair from the scalp is unavailable, a similar quantity of body hair can be used. When discussing head hair, it refers specifically to scalp hair. "Body hair" includes all other hair types (such as facial or underarm hair).

Process Overview

The laboratory analysis of a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's preliminary integration into the lab system. This involves confirming the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is a rapid initial assessment for drugs of abuse. Although Screening is economical for excluding drug use in most samples, valid court submission requires a confirmed positive screen. Any samples suspected to be positive in Screening must undergo secondary confirmation.

In cases where a Screening stage indicates presumptive positivity, additional hair is extracted from the initial sample and readied for Extraction. During this stage, drugs are extracted from the hair at concentrations much lower than those in other methods (like urine or oral fluid), thus making hair drug screening the most intricate method to conduct.

Confirmation of any positive screen is conducted using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptive positive samples are cleaned prior to confirmation where necessary. The process from Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and receives accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
